All benefits at the Centurion, Steward, and Sustainer levels and:

• Naming of the AAO-HNSF headquarters building with approval of the Board of Directors ($5 million gift)
• Naming of the auditorium space housed in the AAO-HNSF headquarters building ($1 million minimum gift)
• A gold medallion with an etched impression of founder Hal Foster, MD to wear at special recognition functions
• Leadership dinner and recognition event in honor of the Visionary and his/her family
• Feature article and photo in special edition Bulletin campaign celebration publication

• A silver medallion with an etched impression of founder Hal Foster, MD to wear at special recognition functions

All endowment donors will be honored through a special permanent commemorative display in the lobby of the AAOHNSF headquarters building

• Listing in special edition Bulletin campaign celebration publication (if gift is committed by September 18, 2010)
• Legacy Circle membership, if participating through a planned gift of $25,000 or more
• Donor wall (web-based and annual meeting) listing
• Millennium Society membership in appropriate category based on level of gift
• Recognition at selected campaign events, at AAO-HNSF meetings, and
• In selected campaign communications

 All benefits at the Sustainer level and:

• Naming opportunities available within the AAO-HNS and AAOHNSF headquarters office with commitment of $250,000 or more
• A bronze medallion with an etched impression of founder Hal Foster, MD to wear at special recognition functions
• Nameplate to be placed on the exterior back of one of the chairs in the auditorium
