We, the undersigned, hereby apply for space at the 2014 Annual Meeting & OTO EXPOSM of the American Academy of Otolaryngology—Head and Neck Surgery Foundation, subject to the rules and regulations governing the exhibition, which are incorporated by reference into this exhibit application, which we accept as part of the agreement. Our insurance documents are valid through October 31, 2014. It is understood that booth assignments will be made by the Foundation in an impartial effort to best serve the interest of all meeting participants. If our selections are not available at the time our application is received, we agree to accept space according to the judgment of the Foundation. The acceptance of our application by the Foundation and the deposit for rental charges constitutes a contract. The Foundation reserves the right to restrict or deny any booth assignment that would compromise the integrity or desirability of the exhibition.

    (*) Only one 10x10 booth is honored at the non-profit rate. Additional booths will be charged at the regular Linear booth rate. Exhibiting for recruiting purposes negates the non-profit rate.

Priority Space Selection Process

Only 2013 OTO EXPOSM exhibitors were part of the onsite Priority Space Selection Process. No other prospects will be able to select space until this process is completed. The Priority Point booth selection opportunity was available from September 29, 2013 to February 2, 2014. Space selection beginning February 3, 2014 will be on a first-come-first-serve basis. A company can utilize their points to gain an advantage and select space only during the Priority Space Selection Process. Each company representative selected to participate in this process will be able to view the live floorplan while making their selection. Once booth space is selected it will be placed on a temporary hold until the exhibit space application and initial deposit are confirmed.

Terms of Payment

The deadline for the 50% payment for applications received during the Priority Space Selection Process is February 3, 2014. After this date, all companies that submitted applications will be liable for 50% of the booth space selected unless a written cancellation is received be 5fore February 3, 2014. After the Priority Space Selection Process is completed, companies submitting applications must also submit at least a 50% deposit with their application. Applications received after May 12, 2014 require payment in full.

Final Payment is due by May 12, 2014

If full payment is not received by this date, booth space may be reassigned and deposit may be returned less 50% of the total cost of the space assigned. All applications received after May 12, 2014 must be accompanied by full payment. No application will be processed without full payment after this date.

Cancellations /Downgrades

Cancellation of space on or before May 12, 2014, will result in a charge equal to 50% of the total cost of the space assigned. The downgrade penalty equates to 50% of your initial booth space which is then added to the cost of your new booth space size if the request is received on or before May 12, 2014.

Refunds

If a cancellation occurs between February 3, 2014 and May 12, 2014 inclusive, a refund in the amount of 50% of the total booth space will be provided, given that full payment has already been received. If only the initial deposit of 50% has been received no refund is granted as each exhibiting company is 50% liable for the space contracted during this time frame. Refunds will not be granted after May 12, 2014 for any reason as each exhibiting company is 100% liable for the space contracted after this date. No exceptions.
